Important Information on COVID-19 Vaccines
NY Family Docs would like to keep you informed of the status of the COVID-19 vaccines availability.
At this time, we do not have the vaccines available in our office and we do not know when we can expect them or in what quantity. However, the vaccine is available at sites throughout the New York City area. New sites and those that are eligible for the vaccine are changing daily.
The best way to stay informed is to visit:
https://vaccinefinder.nyc.gov/ or call (833) 697-4829 (open 7am – 10pm, 7 days/week)
The website and phone number are operated by NYC and lists the sites and the contact phone numbers to schedule an appointment when you meet the eligibility criteria in effect at that time.
If you have questions about the vaccine, please feel free to discuss it with your provider at your next visit with NY Family Docs. We will let you know when the vaccine is available in our offices.
Please continue to wear a facemask and practice social distancing to avoid COVID-19.

The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduce costs. Medical homes foster ongoing partnerships between patients and their personal clinicians, instead of approaching care as the sum of episodic office visits. Each patient’s care is overseen by clinician-led care teams that coordinate treatment across the health care system. Research shows that medical homes can lead to higher quality and lower costs and can improve patient and provider reported experiences of care.
